FedEx Claims Information

Unfortunately, there are times when a package may be lost or damaged in transit. For us to file a claim, we need as much information as possible. That vital information may include the original invoice, an electronic affidavit, pictures of the damaged item(s), and pictures of internal and external packaging. To be fully transparent, we would like to share the claims information directly from FedEx. To file a claim, please use this Delivery Issue Form and include all necessary information. Please do not provide this link to your customers. This should be filled out internally, and if we find it necessary, we will reach out directly to your support team.



FedEx Special Requirements

  • Photos of the package and damaged contents.
  • Supporting documentation
    • Proof-of-value information (ex., a copy of the original invoice from the vendor).
    • A copy of your retail invoice or receipt.
    • The final confirmation screen of an online order with proof of payment.
    • An itemized repair invoice or statement of non-repair.
    • Your appraisals or expense statements.
    • An inspection report that contains images of your shipment’s packaging and damaged contents, if applicable.
  • Serial numbers of damaged or lost merchandise, if applicable.
